Ubuntu Server 16.04.3イメージが書き込まれたUSBドライブでpreseedファイルを使用していますが、preseedファイルを使用するためにisolinux/txt.cfgファイルが変更されています。通常のインストールを行うとインストールが成功するため、preseedファイルの問題だと思います。
